Ciena Healthcare in Michigan is seeking an experienced Care Management Nurse, MDS Nurse, to remain clinically involved in patient care through the MDS/RAI process with autonomy and teamwork.

You will conduct assessments, coordinate care plans, and support PPS meetings while maintaining compliance with Medicare timelines and AANAC guidelines, joining a dedicated team that values communication and ongoing professional growth.

Qualifications

• Must hold RN or LPN license.
• Knowledge of the RAI process; PPS experience is strongly preferred.
• Experience as an MDS Nurse.

Responsibilities

• Completes the MDS, CAA’s and care plans within regulated time frames.
• Assesses resident through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies and uses such information in the assessment and care planning.
• Coordinates, identifies, and/or initiates significant change MDS’.
• Is prepared to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator
• Remains current with the American Association of Nursing Assessment Coordinators (AANAC) requirements.

Qualifications

• Registered Nurse, RN or Licensed Practical Nurse
• Knowledge of the Resident Assessment Instrument (RAI) process, including the principles the Prospective Payment Process (PPS) strongly preferred.
• Experience as an MDS Nurse
