Job summary

Regency at Jackson is seeking an experienced MDS Coordinator to oversee the RAI process and coordinate care plans for residents. You will supervise the Care Management Nurse and MDS Nurse, ensuring timely assessments and accurate documentation within Medicare guidelines.

You will work with an interdisciplinary team to assess resident needs, schedule PPS meetings, and stay current with AANAC requirements, while contributing to a compassionate, collaborative care culture.

Qualifications

• RN licensure required.
• Knowledge of the Resident Assessment Instrument (RAI) and PPS concepts.
• Experience as an MDS Nurse or similar role preferred.

Responsibilities

• Completes the MDS, CAA's and care plans within regulated time frames.
• Coordinates scheduling the RAI process with the interdisciplinary team.
• Assesses resident through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies for assessment and care planning.
• Coordinates and initiates significant changes to MDS as needed.
• Conducts PPS meetings and maintains PPS board for Medicare days and RUGs utilization.
• Keeps current with AANAC requirements.
