Qualifications

• 2+ years of hands-on MDS/RAI experience in long-term care.
• Active RN license in Pennsylvania (unencumbered).
• RAC-CT preferred or strong clinical coding track record.

Responsibilities

• Coordinate and oversee MDS assessments (OBRA/PPS) from admission to discharge with 100% accuracy.
• Lead IDT meetings and develop person-centered care plans.
• Review documentation to maximize appropriate reimbursement.
• Conduct quality audits of medical records and educate staff on documentation.
• Ensure error-free electronic submission of MDS data to CMS.
