Qualifications

• Active LPN license in Pennsylvania (PA) and familiarity with MDS/RAI processes.
• Experience managing or assisting with MDS/RAI in skilled nursing or long‑term care.
• RAC-CT designation or LPNAC equivalent is a plus.
• Analytical mindset with strong attention to detail and compliance.
• Excellent organizational and cross‑departmental communication skills.

Responsibilities

• Coordinate and oversee timely completion of all MDS assessments (OBRA and PPS) from admission to discharge.
• Collaborate with IDT to build person-centered care plans reflecting resident needs.
• Review clinical documentation to ensure it supports care and accurate reimbursement.
• Conduct routine quality reviews on medical records and provide feedback to staff.
• Assist in error-free electronic transmission of MDS data to CMS and implement fixes as needed.

WHAT YOU’LL DO (Your Impact):

• Master the MDS Process: Coordinate and oversee the timely completion of all MDS assessments (OBRA and PPS) from admission to discharge, ensuring 100% accuracy and compliance with Pennsylvania state and federal guidelines.

• Collaborate with the Interdisciplinary Team (IDT): Work closely with nursing, therapy, dietary, and social services to build truly person-centered care plans across our campus that accurately reflect resident needs.

• Maximize Clinical Reimbursement: Review clinical documentation with a sharp eye to ensure it fully supports the care provided, capturing the appropriate resource utilization data for accurate reimbursement.

• Audit & Support: Conduct routine quality reviews on medical records and provide helpful feedback to floor staff regarding the vital importance of complete, precise clinical documentation.

• Manage Transmission Data: Assist in ensuring the smooth, error-free electronic submission of all MDS data to CMS, executing course corrections whenever needed.

WHAT YOU BRING TO THE TEAM:

• The Clinical Standard: An active, unencumbered Licensed Practical Nurse (LPN) license in the state of Pennsylvania. (Must be an LPN to apply).

• MDS/RAI Experience: Hands-on experience managing or assisting with the MDS / RAI process in a skilled nursing or long-term care setting.

• Certification: RAC-CT (Resident Assessment Coordinator Certified) designation or LPNAC equivalent is a massive plus, but a proven track record of clinical coding excellence is highly valued!

• Meticulous Attention to Detail: An analytical mindset with a passion for compliance, data integrity, and complex problem-solving.
