Job summary

Asbury Heights is seeking a Part-Time MDS Coordinator (LPNAC) to join our Pittsburgh campus. The role focuses on leading MDS processes, ensuring compliance and accurate reimbursement while collaborating with the IDT to craft resident-centered care plans.

The candidate must hold an active LPN license in PA and have MDS/RAI experience; RAC-CT is a strong plus. This position offers a day shift with flexible, part-time hours in a premier senior living community.

Qualifications

Active LPN license in Pennsylvania (PA) and familiarity with MDS/RAI processes.
Experience managing or assisting with MDS/RAI in skilled nursing or long‑term care.
RAC-CT designation or LPNAC equivalent is a plus.
Analytical mindset with strong attention to detail and compliance.
Excellent organizational and cross‑departmental communication skills.

Responsibilities

Coordinate and oversee timely completion of all MDS assessments (OBRA and PPS) from admission to discharge.
Collaborate with IDT to build person-centered care plans reflecting resident needs.
Review clinical documentation to ensure it supports care and accurate reimbursement.
Conduct routine quality reviews on medical records and provide feedback to staff.
Assist in error-free electronic transmission of MDS data to CMS and implement fixes as needed.
