Qualifications

• Active unencumbered Licensed Practical Nurse (LPN) license in Pennsylvania.
• Hands-on experience managing or assisting with the MDS / RAI process.
• RAC-CT certification or LPNAC equivalent is a plus.
• Strong communication and organizational skills.

Responsibilities

• Coordinate and oversee completion of all MDS assessments with accuracy.
• Collaborate with interdisciplinary teams to build effective care plans.
• Review documentation for clinical reimbursement accuracy.
• Conduct quality reviews on medical records.
• Ensure smooth electronic submission of MDS data.
