Job summary
Chestnut Hill Rehabilitation and Healthcare Center in Wilkes-Barre, PA is seeking an experienced Registered Nurse Assessment Coordinator (RNAC) to lead the RAI/MDS process, ensure accurate clinical documentation, and support compliant reimbursement. You will collaborate with an interdisciplinary team to maintain MDS 3.0 proficiency and oversee PDPM and UR activities.
You will mentor nursing staff on Medicare criteria, coordinate assessments from admission through weekly UR review, and help
Qualifications
- Current, active RN license.
- Long-term care MDS experience.
- Proficiency in MDS 3.0.
- Knowledge of the PDPM process.
- Strong working knowledge of state and federal regulations.
- Valid Pennsylvania driver’s license.
Responsibilities
- Maintain proficiency in MDS 3.0 and PDPM processes.
- Coordinate and ensure timely MDS submissions for all residents.
- Monitor Medicare schedules and nursing documentation for accuracy.
- Review Case Mix Index (CMI) scores for reimbursement risk.
- Educate staff on Medicare criteria and regulatory changes.
- Oversee UR processes and daily documentation audits.
- Update diagnoses lists in PCC and coordinate OBRA scheduling.
- Provide training to staff as needed.
