Job summary

Medicalodges in Iola, Kansas is seeking a dedicated Charge Nurse/MDS Coordinator to lead the interdisciplinary team in resident assessment and the development of individualized care plans. In this role you will coordinate MDS and Care Plan activities, ensure timely documentation, and monitor compliance with state and federal requirements to deliver compassionate, high‑quality care for our residents.

Responsibilities

• Assess residents and complete the Minimum Data Set and Resident Assessment Protocol; Coordinate MDS and Care Plan Team.
• Implement and monitor individual care plans for residents based on MDS Guidelines.
• Maintain a schedule of due dates for MDS and Care Plans including new admissions and those with significant changes.
• Submit the MDS to the appropriate agency.
