Job summary

Sunnybrook Rehabilitation Center in Raleigh, NC seeks an experienced MDS Coordinator to develop, implement, and coordinate the RAI process. You will ensure compliance with professional standards, physician orders, policies, and applicable regulations.

The role involves collaborating with the care team to maintain accurate assessments and timely documentation. Active RN licensure in NC is required; MDS/RAI expertise and prior long-term care experience are highly valued.

Qualifications

• Active, unrestricted RN license in the practice state.
• Experience with Medicare/Medicaid, OBRA, RAI, and MDS processes preferred.
• Experience in a long-term care facility is a plus.

Responsibilities

• Develop, implement, and coordinate the RAI process.
• Ensure compliance with professional practice standards, physician orders, company policies and procedures, and local, state, and federal regulations.
• Coordinate with clinical and administrative teams to ensure accurate assessments and timely documentation.

As an MDS Coordinator, you will be responsible for developing, implementing and coordinating the Resident Assessment Instrument (RAI) process, ensuring the center stays in compliance with current professional practice standards, physician's orders, company policies and procedures, and local, state, and federal regulations.

Entry Qualifications:

• Active, unrestricted Registered Nurse RN licensure in the state of practice required
• Working knowledge of Medicare and Medicaid programs, LTC payer systems, the Omnibus Budget Reconciliation Act (OBRA), the RAI process, and the MDS process preferred OR prior nursing in a long-term care facility
