Qualifications

• Current registration as an LPN or RN.

Responsibilities

• Assess residents and complete the Minimum Data Set and Resident Assessment Protocol; Coordinate MDS and Care Plan Team.
• Implement and monitor individual care plans for residents based on MDS Guidelines.
• Maintain a schedule of due dates for MDS and Care Plans including new admissions and those with significant changes.
• Submit the MDS to the appropriate agency.
