Job summary

Transitions Healthcare Autumn Grove in Pennsylvania is seeking a Nurse Assessment Coordinator to manage the resident assessment process in accordance with Federal and State regulations.

You will lead the Interdisciplinary Team, coordinate MDS, CAA and Care Plans, and participate in PPS and monthly meetings to ensure compliant, timely billing and accurate documentation.

This role emphasizes case management, staff education on the RAI process, and collaboration to support high-quality resident care.

Qualifications

• Registered Nurse or Licensed Practical Nurse in good standing in PA.
• Two years' experience in institutional resident care preferred.

Responsibilities

• Develop and coordinate the resident assessment process per Federal and State regulations and company policy.
• Provide Case Management and lead PPS, Medicare meetings, and month-end reviews for federal billing.
• Oversee MDS, CAA’s and Care Plans with the Interdisciplinary Team.
• Ensure timely transmission of MDS and verify accuracy across sections.
• Coordinate and participate in the Interdisciplinary Care Plan meetings and updates.
