Job summary

Spring Valley Health & Rehabilitation Center seeks a compassionate MDS Coordinator (RN or LPN) with knowledge of the RAI process and Medicaid eligibility. You will lead MDS coding, coordinate with the interdisciplinary team, ensure timely assessments, and support clinical reimbursement and quality measures.

Qualifications

• Graduate of an accredited school of nursing.
• MDS experience preferred; will educate and grow skill set.
• RN or LPN with a current Missouri license.
• CPR certified.
• Excellent written, verbal, and interpersonal skills.

Responsibilities

• Understand regulations related to RAI process (federal/state and MDS RAI User Manual).
• Participate in pre-admission to obtain information for MDS/Case Mix coding.
• Collaborate with Director of Nursing to ensure accurate nursing documentation and risk assessments.
• Lead MDS coding, file timely submissions, and manage the MDS department.
• Provide staff training on the RAI process and documentation guidelines.
• Monitor QA/QM reports and act on follow-up recommendations.
• Coordinate with IDT and attend relevant meetings.
