You will ensure accurate resident assessments, plan individualized care, maintain regulatory compliance, and support quality outcomes with an interdisciplinary team.

Qualifications

• Current Colorado RN or LPN license in good standing (or compact state license, if applicable).
• CPR Certification.
• MDS experience preferred.
• Long-term care experience preferred.
• Strong organizational, communication, and documentation skills.
• Ability to work collaboratively with an interdisciplinary team.
• Passion for delivering exceptional resident-centered care.

Responsibilities

• Coordinate and complete the MDS assessment process in accordance with state and federal regulations.
• Develop and maintain individualized resident care plans with the interdisciplinary team.
• Ensure accurate and timely completion of all MDS documentation.
• Monitor assessment schedules and regulatory deadlines.
• Review clinical documentation to support quality outcomes and appropriate reimbursement.
• Collaborate with nursing, therapy, dietary, social services, and other departments to ensure comprehensive resident care.
• Assist with survey readiness and regulatory compliance.
• Maintain compliance with facility policies and procedures.
• Perform other duties as assigned.
