Qualifications

• Must hold a Maryland LPN license.
• MDS certification or proven MDS assessment experience preferred.
• Minimum 2 years clinical experience; at least 1 year in long‑term care.

Responsibilities

• Demonstrate commitment to NLCS' core values in all aspects of work.
• Support NLCS policies and code of conduct.
• Coordinate individualized care plans using interdisciplinary team.
• Review MDS assessments and care plans for proper implementation.
• Participate in resident care plan conferences.
• Maintain MDS schedule and timeliness of assessments.
• Transmit CMS assessments and reconcile transmissions.
