Job summary

National Lutheran Communities & Services is seeking a Licensed Practical Nurse (LPN) MDS Coordinator in Rockville, MD. This role focuses on accurate MDS assessments, care planning, and reimbursement processes to support high-quality resident care.

You will collaborate with an interdisciplinary team to ensure timely documentation, regulatory compliance, and ongoing quality improvement while upholding NLCS’s mission to honor and enrich the lives of older adults.

Qualifications

• Current LPN license in Maryland.
• Minimum 1 year of nursing experience in long-term care, skilled nursing, or related setting.
• Strong knowledge of nursing documentation, resident assessment, and care planning.
• Excellent organizational, analytical, and communication skills.
• Ability to read and speak English.

Responsibilities

• Coordinate and complete the MDS assessment process per regulations.
• Collaborate with interdisciplinary team to gather assessment data.
• Develop and update individualized care plans based on assessments.
• Monitor assessment schedules and ensure timely documentation submission.
• Review clinical documentation for accuracy and regulatory compliance.
• Support quality measures and performance improvement initiatives.
• Serve as a clinical resource for MDS regulations and reimbursement processes.
• Work with nursing leadership to ensure regulatory compliance and excellence in resident care.
