Job summary

The Village at Rockville, part of National Lutheran Communities & Services, is seeking a dedicated Licensed Practical Nurse (LPN) MDS Coordinator to join our clinical team. This role focuses on accurate resident assessments, care planning, and regulatory compliance to support quality care and accurate reimbursement.

You will collaborate with an interdisciplinary team to gather data, develop individualized care plans, and monitor assessment schedules, ensuring timely documentation and regulatory

Qualifications

• Current Maryland LPN license.
• Minimum 1 year nursing experience in long-term care or skilled nursing.
• Strong knowledge of nursing documentation, resident assessment, and care planning.
• Excellent organizational, analytical, and communication skills.
• Ability to manage multiple priorities and meet deadlines.
• Ability to read and speak English.

Responsibilities

• Coordinate and complete the MDS assessment process in accordance with federal, state, and organizational regulations.
• Collaborate with interdisciplinary team members to gather assessment data and ensure comprehensive resident evaluations.
• Develop and update individualized care plans based on resident needs and assessment findings.
• Monitor assessment schedules and ensure timely completion and submission of all required documentation.
• Review clinical documentation for accuracy, completeness, and regulatory compliance.
• Support quality measures and performance improvement initiatives.
• Serve as a clinical resource for MDS-related regulations, documentation requirements, and reimbursement processes.
• Work closely with nursing leadership to promote excellence in resident care and regulatory compliance.
