Job summary

Rocky River Gardens in Cleveland, OH is seeking an experienced full-time RN/LPN MDS Coordinator to coordinate the RAI process, ensuring timeliness and completeness of the MDS, CAAs, and Interdisciplinary Care Plan.

The role includes supervising MDS input, state transmission, care plan conferences, and QM meetings, with a focus on regulatory compliance and smooth care coordination. A $5,000 sign-on bonus is offered to qualified applicants.

Qualifications

• Must hold a Nursing Degree from an accredited college or university.
• A current, unencumbered, active RN/LPN license and CPR
• At least 2 years’ experience in Long Term Care as an MDS Coordinator.

Responsibilities

• Monitors and guides the completion of assessments in a timely manner
• Involvement and supervision of inputting of MDS assessments, the transmission to the state, and obtains receipt of the validation
• Schedules and facilitates care plan conferences Facilitates monthly QM meetings
• Manages Care Planning Process
