Wurtland Nursing and Rehab is seeking an experienced MDS Coordinator (RN or LPN) to manage MDS and care plans for residents, ensuring timeliness and compliance with state requirements. The role emphasizes coordination with the care team and accurate documentation.

The candidate should be proficient with MDS 3.0, care planning, and data entry. Excellent reading, writing, and English communication are essential for resident safety and quality care.

Formación

• Must hold current RN/LPN license in the state of employment; license active and in good standing.
• Experience in Long Term Care Medical Records.
• Knowledge of MDS 3.0 and care planning processes.

Responsabilidades

• Conduct and coordinate the MDS and Care Plan as outlined by the facility’s policies and procedures.
• Inform care team when a care plan is due and ensure completion by all team members.
• Develop and/or revise resident care plans quarterly or with any significant change in condition.
