Qualifications

• Current Colorado RN or LPN license in good standing, or compact license if applicable.
• Current CPR Certification.
• Previous MDS experience preferred.
• Long-term care experience preferred.
• Strong organizational and time-management skills.
• Excellent communication and documentation abilities.
• Ability to work effectively with an interdisciplinary team.
• A passion for providing exceptional care and improving the lives of residents.

Responsibilities

• Coordinating and completing the MDS assessment process in accordance with state and federal regulations.
• Developing and maintaining individualized resident care plans alongside the interdisciplinary team.
• Ensuring all MDS documentation is accurate, complete, and submitted on time.
• Monitoring assessment schedules and important regulatory deadlines.
• Reviewing clinical documentation to support quality outcomes and appropriate reimbursement.
• Collaborating with nursing, therapy, dietary, social services, and other departments.
• Supporting survey readiness and ongoing regulatory compliance.
• Helping ensure the highest standards of resident-centered care throughout the facility.
• Maintaining compliance with facility policies and procedures.
• Performing other duties as assigned.
