As the MDS Coordinator, you will play an essential role in coordinating resident assessments and ensuring high-quality, individualized care.
Your responsibilities will include:
Coordinating and completing the MDS assessment process in accordance with state and federal regulations
Developing and maintaining individualized resident care plans alongside the interdisciplinary team
Ensuring all MDS documentation is accurate, complete, and submitted on time
Monitoring assessment schedules and important regulatory deadlines
Reviewing clinical documentation to support quality outcomes and appropriate reimbursement
Collaborating with nursing, therapy, dietary, social services, and other departments
Supporting survey readiness and ongoing regulatory compliance
Helping ensure the highest standards of resident-centered care throughout the facility
Maintaining compliance with facility policies and procedures
Performing other duties as assigned
Current Colorado RN or LPN license in good standing, or compact license if applicable
Current CPR Certification
Previous MDS experience preferred
Long-term care experience preferred
Strong organizational and time-management skills
Excellent communication and documentation abilities
Ability to work effectively with an interdisciplinary team
