Job summary

The Greens at Pinehurst in Pinehurst, NC is seeking an experienced RN/LPN MDS Coordinator for a full-time role. The position requires expertise in the MDS process, care planning, and regulatory compliance to ensure accurate assessments and high-quality resident care.

Qualifications

• Active RN or LPN license in North Carolina
• Minimum 3 years of MDS Coordinator experience in Long-Term Care
• Strong knowledge of MDS assessments, Case-Mix, Medicare and Medicaid reimbursement
• Familiarity with federal and state regulations governing skilled nursing facilities
• Excellent organizational, communication, and critical-thinking skills
• RAC-CT Certification Preferred

Responsibilities

• Coordinate and oversee the RAI/MDS assessment process, ensuring accuracy, completeness, and timely submissions
• Monitor completion of MDS assessments, CAAs, and interdisciplinary care plans
• Ensure accurate transmission of MDS assessments and obtain validation reports
• Schedule and facilitate interdisciplinary care plan conferences
• Participate in monthly Quality Measure (QM) meetings
• Manage the care planning process while maintaining regulatory compliance
• Collaborate with nursing leadership to support quality resident outcomes
