Lillington Health & Rehabilitation Center in Lillington, NC is seeking a qualified MDS Coordinator to ensure accurate MDS assessments and coordinate care plans with the interdisciplinary team. Qualifications

• Active, unencumbered RN licensure in North Carolina.
• LPN license with relevant experience will be considered.
• MDS experience in skilled nursing/long-term care preferred.
• Proficient with Microsoft Office Suite.
• Excellent communication, highly organized, and detail oriented.
• Ability to work independently or as part of a team.
• Salaried/exempt position.

Responsibilities

• Complete and submit accurate MDS assessments.
• Oversee care plan process including conferences.
• Ensure ARD stays within Medicare & Medicaid guidelines.
• Participate in monthly Quality Measure meetings.
• Work with nursing leadership to identify and resolve opportunities in documentation.
• Assist interdisciplinary team in making appropriate Medicare coverage decisions.
