Job summary

The Greens at Erwin in the United States is seeking an RN or LPN MDS Coordinator to join our skilled nursing team. You will oversee the Resident Assessment Instrument (RAI) process, coordinate MDS assessments, CAAs, and interdisciplinary care plans, ensuring accuracy and timeliness.

We offer MDS training, no on-call shifts, and growth opportunities within a supportive regional MDS team. This full-time role emphasizes compliance with Medicare/Medicaid guidelines, QA involvement, and collaboration

Qualifications

• RN or LPN with active license and LTC experience preferred.
• MDS knowledge/experience is desirable; willing to train.
• Strong organizational and communication skills required.

Responsibilities

• Oversee the RAI/MDS process, ensuring timely and accurate submissions.
• Ensure compliance with Medicare/Medicaid, PDPM, OBRA and related regulations.
• Develop individualized care plans and coordinate interdisciplinary team.
• Schedule and lead care plan conferences and QM meetings.
