Perry Creek Health & Rehabilitation Center in Raleigh, NC is seeking a qualified candidate to join our team as a MDS Coordinator. This negotiable role focuses on ensuring accurate MDS assessments and coordinating with the care team to develop comprehensive plans of care.

Active RN licensure in North Carolina is required (LPN considered with relevant experience). Strong MS Office skills, communication, organization, and independence are essential for success in this salaried position.

Qualifications

• Active, unencumbered RN licensure in North Carolina.
• LPN licensure considered with relevant experience.
• MDS experience in Skilled Nursing/Long-Term Care preferred.

Responsibilities

• Complete and submit accurate MDS assessments.
• Oversee care plan process including conferences.
• Ensure ARD compliance with Medicare/Medicaid guidelines.
• Participate in monthly Quality Measure meetings.
• Collaborate with nursing leadership to improve documentation.
• Assist interdisciplinary team in Medicare coverage decisions.
