Job summary

Cascadia Healthcare is seeking an MDS Coordinator (RN) to ensure timely, accurate assessments and care plans that meet federal and state guidelines. You will identify resident needs, coordinate interdisciplinary teams, and communicate with families and payors to ensure high-quality, compliant care.

The role requires RN licensure, CMS/MDS expertise, and strong leadership and communication skills to support facility operations and clinical outcomes.

Qualifications

• Knowledge of CMS guidelines for MDS
• Ability to implement Nursing department programs/goals
• Knowledge of nursing practices, laws and guidelines
• Leadership ability and staff supervision
• CPR certification upon hire and maintained
• Excellent written and verbal communication, multitasking

Responsibilities

• Manage MDS assessments based on CMS guidelines.
• Coordinate, record, update and monitor all MDS.
• Participate in clinical meetings and care conferences.
• Coordinate family and resident communications with staff and payors.
• Review quality metrics and support quality assurance action planning.
• Ensure timely development of resident assessment and care plans.
• Apply MDS/PPS rules using Utilization Guidelines for assessments.
