Job summary

Midtown Center for Health and Rehabilitation in Memphis, TN is seeking an MDS Coordinator to lead MDS/RAI processes within our nursing home. You will collaborate with the interdisciplinary team to ensure accurate documentation and compliance with MDS 3.0, Medicaid and Medicare requirements.

Ideal candidates hold an LPN or RN license, have prior MDS experience in a skilled nursing facility, and possess strong chart audit and clinical support skills.

Qualifications

• Current State license as an LPN or RN.
• Previous MDS experience in Skilled Nursing Facilities.
• Must know MDS 3.0 Experience.
• Knowledge of Medicaid and Medicare.
• Must have long-term care experience.

Responsibilities

• Ensure utilization of the interdisciplinary rehab team process in the formulation of MDS/RAI's.
• Observe and review documentation, make recommendations, assist with chart audits.
• Provide clinical support and direction to appropriate resource materials.
