Job summary

Smoky Mountain Health & Rehabilitation Center in Pigeon Forge, TN is seeking an experienced RN or LPN MDS Coordinator to join our team. The role focuses on coordinating the RAI, MDS, CAAs, and interdisciplinary care plans to support accurate assessments and compliant documentation.

Responsibilities include overseeing the MDS process, ensuring timely submissions, supporting care planning, and leading QM meetings.

Qualifications

Active Tennessee RN or LPN license
Nursing degree or diploma from accredited program
Current CPR certification
Minimum of 2 years Long-Term Care MDS Coordinator experience
RAC-CT certification preferred
Experience with Medicare, Medicaid, reimbursement, and Case-Mix

Responsibilities

Coordinate and oversee MDS assessments within required timeframes
Guide the interdisciplinary team through the RAI/MDS process
Review MDS for accuracy, completeness, regulatory compliance
Manage submission and validation of MDS assessments
Coordinate resident care planning process
Schedule and facilitate interdisciplinary care plan conferences
Ensure care plans reflect resident needs and goals
Lead monthly QM meetings
Monitor quality measures and identify improvement opportunities
Maintain compliance with federal/state/local regulations
Collaborate with nursing, therapy, dietary, social services
Support accurate reimbursement through proper assessment, documentation, and coding
