Job summary

Collierville Nursing and Rehabilitation is seeking an experienced MDS Coordinator (LPN or RN) to lead MDS/RAI assessments, care planning, and regulatory compliance in a skilled nursing setting.

You will collaborate with an interdisciplinary team, review care documentation, and ensure Medicare/Medicaid readiness, with emphasis on accuracy and resident-centered care.

A current LPN/RN license, MDS certification, and MDS 3.0 experience in long-term care are required.

Qualifications

• Current license as LPN or RN.
• Certification in MDS.
• MDS 3.0 experience in Skilled Nursing Facilities.
• Knowledge of Medicaid and Medicare.
• Long-term care experience.

Responsibilities

Requirements of the MDS Coordinator

• Current license as LPN or RN
• Certification in MDS
• Previous MDS experience gained in Skilled Nursing Facilities
• Must have MDS 3.0 Experience
• Knowledge of Medicaid and Medicare
• Must have long-term care experience
