Caretel of St. Joseph is seeking an MDS Coordinator (RN/LPN) to lead the MDS data management and utilization processes in a Skilled Nursing setting.

You will ensure accurate coding, timely assessments, and collaboration with the nursing team to optimize Medicare and PDPM outcomes. The role requires an RN or LPN license, a bachelor’s degree or state-certified program, and CPR certification, with strong attention to regulatory requirements and documentation.

• Leads MDS data management to meet federal/state guidelines
• Coordinates resident assessments and care plans with accuracy
• Ensures timely MDS coding and data transmission to CMS
• Collaborates with Interdisciplinary Team and families to improve care quality
