Job summary

Apploi in Chicago seeks an MDS Coordinator to assess and coordinate resident MDS processes, ensuring compliance with State and Federal regulations and facility policies. Reports to the Director of Nursing.

The role requires an active Illinois RN or LPN license, strong English communication, and the ability to lead interdisciplinary care conferences and Medicare meetings. You will ensure timely documentation, correct MDS scoring, and optimal reimbursement within regulatory requirements.

Qualifications

• Active Illinois RN or LPN license required.
• Strong knowledge of MDS/RAI guidelines and regulations.
• Ability to coordinate multidisciplinary team meetings.
• Excellent documentation and record-keeping abilities.
• Proficient with computer-based charting and data entry.

Responsibilities

• Coordinate in conjunction with the Care Plan Coordinator the completion of each resident's MDS in compliance with all State and Federal requirements.
• Calculate triggers and develop resident assessment protocols for initiation of care plan.
• Ensure that MDS's are completed in a timely manner and accurately identify each resident's level of functioning in accordance with RAI manual guidelines.
• Ensure that each MDS accurately identifies the lowest level of functioning allowable to maximize reimbursement opportunities as allowable by Federal and State law.
• Ensure that CNAs, nurses and IDT disciplines have accurately completed required clinical record documentation in order to correctly complete MDS scoring. Ensure that this record completion is performed in a timely manner.
