Qualifications

• Active RN or LPN license in good standing.
• Minimum 6 months of experience as an MDS Coordinator.
• Experience in Skilled Nursing/Rehabilitation preferred.
• RAC-CT or RNAC certification preferred.
• Detail-oriented and committed to excellence.

Responsibilities

• Coordinate and participate in completing the MDS and care plans in compliance with regulations.
• Monitor Medicare and Managed Care residents and collaborate with the interdisciplinary team.
• Ensure accuracy of resident assessments to maximize reimbursement and care quality.
• Implement new ideas and processes to improve resident outcomes.
• Maintain compliance with evolving regulations and best practices.
