Diversicare is seeking an MDS Coordinator to guide the MDS process in our Mississippi facilities. The role requires RN licensure and familiarity with MDS 3.0, data entry, and care coordination with interdisciplinary teams.

Qualifications

• RN license required or in-state equivalent.
• 2 years MDS experience preferred.
• Experience in Long Term Care preferred.
• Proficient in data entry and word processing; knowledge of MDS 3.0.

Responsibilities

• Coordinate the RAI/MDS processes for residents.
• Collaborate with interdisciplinary team to assess patient needs.
• Lead care coordination meetings and track MDS schedules.
• Ensure accurate coding and timely submission of MDS data.
