Job summary

Complete Care at Harrington Court in Connecticut is hiring a MDS Coordinator (RN or LPN) to oversee resident assessments and ensure timely completion. The role requires MDS experience, knowledge of Case-Mix and Medicare PPS, and strong computer skills.

This is a full-time day-shift position located in CT. You will monitor assessments, supervise data entry, transmit MDS to the state, and coordinate quarterly and annual care plan conferences with the Director of Nursing and Administration.

Qualifications

• MDS experience is required.
• Active RN or LPN license in CT.
• Strong knowledge of Case-Mix, Medicare PPS and medical reimbursement.
• Ability to work independently or as part of a team.
• Computer literacy and familiarity with facility software.

Responsibilities

• Monitor and guide completion of resident assessments in a timely manner.
• Involvement and supervision of inputting MDS assessments.
• Transmit MDS assessments to the state and obtain receipt of validation.
• Schedule and facilitate care plan conferences on a quarterly and annual basis.
• Other duties as assigned by Director of Nursing and Administrator.
