Job summary

Complete Care at Green Knoll in New Jersey is seeking an RN MDS Coordinator to lead the MDS process and support the clinical team.

You will conduct MDS assessments, ensure regulatory compliance, and collaborate with interdisciplinary staff to develop individualized care plans. This role offers full-time hours, on-call requirements, and opportunities for advancement with health, dental, vision, and PTO benefits.

Qualifications

• Associate or Bachelor's degree in Nursing (ADN/BSN).
• Active, unencumbered New Jersey RN license.
• Experience in long-term care preferred.
• Minimum 2 years as an MDS Coordinator in long-term care.
• LPN with MDS experience may be considered.

Responsibilities

• Conduct and coordinate MDS assessments per regulations.
• Collaborate with interdisciplinary teams for resident assessments.
• Ensure accuracy of MDS for care planning and compliance.
• Stay updated on MDS regulation changes and implement updates.
• Participate in care planning meetings and develop individualized plans.
• Monitor resident progress and update assessments as health changes occur.
• Work with nursing/administrative staff on billing and reimbursement.
• Provide staff education on MDS processes and documentation.

What You’ll Do:

Registered Nurse-RN-MDS Coordinator Qualifications and Responsibilities:

• Associates or Bachelor's degree in Nursing
• Active, unencumbered New Jersey RN license
• Previous long-term care experience is preferred
• 2 years proven experience as an MDS Coordinator in a long-term care setting.
• Conduct and coordinate the Minimum Data Set (MDS) assessments for residents in accordance with federal and state regulations.
• Collaborate with interdisciplinary teams to gather information for comprehensive resident assessments.
• Ensure accuracy and completeness of MDS assessments to support resident care planning and regulatory compliance.
• Stay updated on changes in regulations related to MDS assessments and implement necessary adjustments.
• Actively participate in care planning meetings and contribute to the development of individualized resident care plans.
• Monitor and track resident progress, updating assessments as needed to reflect changes in health status.
• Work closely with nursing and administrative staff to facilitate accurate billing and reimbursement processes.
• Provide education and training to staff on MDS assessment processes and documentation requirements.
• LPN with MDS experience MAY be considered
