Job summary

Complete Care at Monmouth, part of Complete Care, Inc., is seeking a Registered Nurse with MDS coordination expertise to ensure accurate assessments and individualized care plans for residents.

You will work with an interdisciplinary team to drive quality outcomes, maintain compliance with federal and state MDS regulations, and educate staff on MDS processes. Full-time day shift with competitive benefits and PTO in a supportive, resident-centered environment.

Qualifications

  • Associate's or Bachelor's Degree in Nursing as required.
  • Current, unrestricted NJ RN license.
  • Long-term care experience preferred.
  • Experience as RN-MDS Coordinator or MDS assessment in skilled nursing preferred.
  • Strong clinical assessment, communication, and organizational skills.
  • Knowledge of federal and state MDS regulations.
  • Ability to work collaboratively with interdisciplinary teams.

Responsibilities

  • Coordinate and complete accurate MDS assessments in accordance with federal and state guidelines.
  • Partner with nursing, therapy, social services, and other departments to develop individualized resident care plans.
  • Ensure documentation supports quality resident care, regulatory compliance, and reimbursement accuracy.
  • Monitor assessment schedules and update MDS documentation as residents' conditions change.
  • Participate in care plan meetings and contribute clinical expertise to improve resident outcomes.
  • Educate and support clinical staff on MDS processes, documentation standards, and regulatory updates.
  • Work closely with leadership to maintain compliance and promote continuous quality improvement.
  • Help ensure Complete Care at Monmouth continues to provide the highest level of resident-centered care.

Registered Nurse-RN-MDS Coordinator Qualifications

  • Associate's or Bachelor's Degree in Nursing
  • Current, unrestricted New Jersey Registered Nurse (RN) license
  • Previous long-term care experience preferred
  • Experience as a Registered Nurse-RN-MDS Coordinator or MDS assessment experience in a skilled nursing setting preferred
  • Strong clinical assessment, communication, and organizational skills
  • Knowledge of federal and state MDS regulations
  • Ability to work collaboratively with interdisciplinary teams

Registered Nurse-RN-MDS Coordinator Responsibilities
