Job summary

Majestic Care of Jefferson Pointe seeks an MDS Coordinator (RN) to supervise and coordinate MDS and care plans for residents, ensuring timely, accurate submissions per state requirements.

You will lead care plan meetings, monitor MDS timelines, and collaborate across departments, maintaining resident rights, privacy, and compliance with PPS/CAAS guidelines. The role requires an active RN (or LPN) license, Long Term Care experience, and strong computer skills including MDS 3.0.

Qualifications

• Must hold active RN or LPN license in the state of employment and be in good standing.
• Experience in Long Term Care preferred.
• Proficient with MDS 3.0 and basic computer tasks (data entry/word processing).

Responsibilities

• Coordinate and conduct MDS and Care Plans per facility policies.
• Inform care team when a plan is due and ensure completion of MDS portions.
• Develop or revise resident care plans quarterly or with changes in condition.
• Handle PPS and state MDS assessments; ensure CATs/CAAS completed.
• Coordinate care plan meetings and involve team members.
• Submit MDS timely to state/federal agencies and maintain schedules.
• Ensure residents' rights, confidentiality, and privacy in MDS processes.
• Maintain cross-department communication to keep plans current.
