Majestic Care of Manchin is seeking an MDS Coordinator (RN) to supervise MDS and care plans and ensure timely, compliant completion for residents. This on-site role requires strong knowledge of MDS 3.0, PPS, and state/federal guidelines, with collaboration across care teams.

The position emphasizes accuracy, confidentiality, and proactive communication within the facility, offering competitive pay and Majestic Difference benefits.

Formación

• Must hold current RN or LPN license in the state of employment.
• Experience in Long Term Care preferred.
• Proficient in data entry and Word processing; working knowledge of MDS 3.0.
• Knowledgeable of nursing and medical practices and state/federal guidelines for skilled nursing facilities.
• Must possess ability to plan, organize, develop, implement and interpret programs, goals and procedures.
• Must hold an active CPR license or ability to obtain within 30 days.

Responsabilidades

• Supervise and coordinate MDS and care plans for residents per facility policies.
• Inform care team when care plans are due and ensure completion of MDS portions.
• Develop/revise resident care plans quarterly or with significant change in condition.
• Manage PPS and state MDS assessments; ensure all MDS, CATs, CAAs are completed.
• Coordinate care plan meetings and encourage team participation.
