Surrey Place Healthcare & Rehabilitation in Bradenton, FL is seeking a licensed RN to serve as MDS Coordinator. The role involves coordinating the RAI process, completing MDS, and developing the Interdisciplinary Plan of Care, under CMS guidelines and regulatory requirements.

The MDS Coordinator will lead the daily PPS/Medicaid case-mix meetings, ensure OBRA compliance, and collaborate with the care team to optimize patient outcomes. Florida AHCA background screen is required.

Qualifications

• Must be a Florida-licensed RN.
• Responsible for coordinating the RAI process including completion of an accurate MDS and development of the Interdisciplinary Plan of Care.
• Adheres to CMS guidelines and facility policies.

Responsibilities

• Maintain MDS status for each patient/resident per OBRA, CMS, and state/federal guidelines.
• Schedule required assessments (OBRA, Medicare PPS, Medicaid).
• Coordinate and communicate schedules to departments and families.
