Job summary

Westchester Gardens Health & Rehabilitation in Clearwater, FL, seeks an experienced MDS Coordinator (RN) to lead the Resident Assessment Instrument process in compliance with state and federal requirements. You will coordinate MDS/CAA/CAT, work with IDT, and ensure care plans reflect resident needs.

The role requires an active RN license in Florida, at least 1 year of RAI experience, and preference for 2 years in a long-term care setting.

Qualifications

• Must meet all applicable state and federal requirements.
• 1 year of experience in the RAI process.
• 2 years long term care facility care experience preferred.
• Possess a current and active license to practice as an RN in this state.

Responsibilities

• Coordinate development and completion of the MDS assessment per rules, regulations, and guidelines.
• Manage reimbursement processes and collaborate with business office and therapy department.
• Input and update policies and procedures governing MDS and care plans.
• Participate in facility surveys and inspections by government agencies.
• Ensure compliance with the Resident Assessment Instrument (RAI) process.
• Collaborate with the Interdisciplinary Team to complete MDS and care plans for each resident.
• Coordinate written care plans with measurable objectives and timelines.
• Ensure staff understand and implement care plans.
