Job summary

Proforma Health Inc is seeking an experienced MDS Coordinator (RN license required) based in Fayetteville, NC. The role focuses on managing MDS due dates, coordinating interdisciplinary teams, and ensuring accurate data submission to the State authorities.

Responsibilities include developing Resident Care Plans with the care team, leading RCP meetings, correcting records, and ensuring compliance with state guidelines. Background checks and regulatory knowledge are essential.

Qualifications

• Current RN license from an accredited school and the North Carolina Board of Nursing.
• Ability to follow policies, procedures, and regulatory requirements.
• Experience with MDS-RAP and Care Planning; familiarity with RUG-IV and MDS 3.0 preferred.

Responsibilities

• Maintain and adhere to MDS due date schedules.
• Coordinate multidisciplinary efforts for MDS completion.
• Review data for accuracy and ensure timely submission and approval by the State.
• Correcting rejected records and preparing for re-submission.
• Completing CAAs as per state guidelines.
• Developing Resident Care Plans (RCP) with interdisciplinary teams within designated timeframes.
• Scheduling and leading RCP meetings, communicating plans to staff, and ensuring compliance.
• Collaborating with departments to ensure understanding and adherence to RCPs.
• Performing additional duties as directed by supervisors.
