Asheboro Rehabilitation and Healthcare Center in Asheboro, NC is seeking an MDS Coordinator to join our dedicated nursing team. This full-time role focuses on coordinating the MDS process, developing care plans, and ensuring regulatory compliance.

The ideal candidate will hold a current nursing license (RN/LPN) and CPR certification, with knowledge of long-term care billing and documentation.

Qualifications

• Must hold a current nursing license (RN or LPN).
• CPR certification required.
• Knowledge of MDS processes and long-term care documentation.

Responsibilities

• Keep abreast of current federal and state regulations and professional standards.
• Assist with development of comprehensive care plans with the MDS.
• Coordinate, manage, and monitor the written plan of care for each resident.
• Assist nursing management with coordination, management, and review of nurse's notes.
• Monitor resident status changes and ensure timely nursing or clinical involvement.
• Ensure MDS and support documentation accurately reflect the resident and meet regulatory requirements.
• Perform regular audits of documentation for accuracy.
• Assist with the discharge process.
• Complete necessary forms and reports.
• Maintain infection control practices and safety precautions.
• Maintain confidentiality of health information.
• Report hazardous conditions and equipment issues.
• Maintain resident dignity and respectful interactions.
• Respond to call lights promptly.
• Communicate effectively with residents, families, and staff.
• Attend departmental meetings and trainings.
