Signature HealthCARE at Colonial Rehab & Wellness Center in Bardstown, KY seeks an MDS Candidate. This role focuses on detailed resident assessments, data collection from medical records, and coordinating with families and the interdisciplinary team.

You will participate in PPS meetings, ensure compliance with federal billing via Plan of Care reviews, and mentor others on the RAI process. No call rotation or weekend requirements; MDS growth tracks available.

Qualifications

• Must be a Registered Nurse with current state licensure.
• Minimum 3 years clinical experience in a healthcare setting; long-term care preferred.
• Minimum 1 year of MDS experience.
• No call rotation or weekend requirement.
• MDS growth tracks available.

Responsibilities

• Collect information to complete the MDS using the medical record, bedside assessment, and interviews with staff, residents, and families.
• Attend Daily PPS meetings, weekly Medicare meetings, and month-end reviews to ensure federal billing requirements are met.
• Educate the interdisciplinary team on the RAI process and changes.
