Job summary

Signature HealthCARE at Heritage Hall Rehab & Wellness invites an experienced Registered Nurse to join our MDS team. You will collect information to complete the MDS, participate in PPS and Medicare meetings, and review plans of care quarterly to ensure accurate updates.

The ideal candidate has a current RN license, 3+ years in healthcare, and at least 1 year of MDS experience. We offer growth, stability, and a supportive work environment.

Qualifications

Must hold an active RN license in the state.
Minimum three years of clinical experience in a health care setting; long-term care preferred.
Minimum one year of MDS experience required.

Responsibilities

Collect information to complete the MDS using the medical record, bedside assessment, and staff/family interviews.
Participate in Daily PPS meetings, weekly Medicare meetings, and month-end reviews to ensure billing requirements are met.
Provide ongoing education of the RAI process to the interdisciplinary team and care staff.
