Job summary

Signature HealthCARE North Hardin Rehab & Wellness Center in Radcliff, KY is seeking a licensed practical nurse with MDS experience to lead data collection for resident assessments and coordinate closely with families and staff.

You will participate in PPS and Medicare meetings, review plans quarterly, and ensure comprehensive assessments reflect changes. This full-time role offers stability in a renowned SNF network with resident-centered care.

Qualifications

• Licensed Practical/Vocational Nurse with current state licensure.
• Minimum three years of clinical experience in a health care setting; long-term care preferred.
• Minimum one year of MDS experience.

Responsibilities

• Collect information to complete the MDS using the medical record, bedside assessment, and staff, resident and/or family interviews.
• Participate in Daily PPS meetings, weekly Medicare meetings, and month end meetings to assure federal billing requirements are met.
• Review Plan of Care at least quarterly and with each Comprehensive Assessment to assure changes during the quarter are included and updated.
