Job summary

Otterbein SeniorLife is seeking an MDS Coordinator (RN) to lead the development, completion, and transmission of resident assessments and care plans. You will ensure documentation and coding comply with regulations and support appropriate reimbursement, while serving residents, families, and the care team.

In this role you will collaborate with an interdisciplinary team, stay current on regulations, and participate in quality improvement activities.

Qualifications

• Graduate of approved nursing program - RN.
• Current, unrestricted Ohio RN license in good standing.
• At least three years of MDS experience in long-term care or nursing home.

Responsibilities

• Coordinate and complete accurate, timely MDS assessments in accordance with federal and state requirements.
• Create, maintain, and update individualized, person-centered care plans with the interdisciplinary team.
• Review medical records and obtain information to support accurate MDS coding and care planning.
• Monitor and guide contributions from nursing and other disciplines to ensure complete and accurate plans.
