Qualifications

• RN license required and active in Ohio

• Proficient in electronic medical records and care coordination

Responsibilities

• Coordinate development, completion and transmission of assessments
• Case manage care plans including rehabilitative programs
• Comply with regulatory requirements and reimbursement processes
• Lead and deliver high-quality services in a person-centered environment
