Otterbein SeniorLife is seeking an MDS Coordinator (RN) to lead the resident assessment process, ensuring accurate MDS coding and person-centered care planning. You will collaborate with an interdisciplinary team to develop and update care plans that reflect residents' needs, goals, and preferences, while staying compliant with regulations.

Qualifications

• RN licensure in Ohio is required.
• At least 3 years of MDS experience in long-term care or nursing home settings preferred.
• Post-acute and geriatric nursing experience preferred.

Responsibilities

• Coordinate and complete accurate MDS assessments with federal/state requirements.
• Create, maintain, and update individualized care plans with the interdisciplinary team.
• Review medical records and interview residents/families to support MDS coding and planning.
• Ensure documentation supports assessment findings, coding, services, and reimbursement.
• Coordinate data entry, submission, and transmission within required timeframes.
• Guide contributions from nursing and other disciplines for complete, accurate plans.
