Job summary

Iliff is seeking an MDS Coordinator (RN) to lead the resident assessment process in a skilled nursing setting. You will oversee the MDS/CAA and care planning, ensuring accuracy, regulatory compliance, and timely transmission across payer sources.

This role requires advanced clinical judgment, 3+ years of clinical experience, and strong collaboration with an interdisciplinary team to drive quality and reimbursement accuracy while supporting audits and quality initiatives.

Qualifications

  • Current, active RN license in the state
  • Minimum three years of clinical experience; long-term care preferred
  • Strong working knowledge of RAI/MDS regulations and reimbursement systems
  • Excellent analytical, organizational, and problem-solving skills
  • Ability to lead interdisciplinary collaboration and meet regulatory deadlines

Responsibilities

  • Oversee and coordinate the full RAI/MDS, CAA, and care planning process
  • Ensure assessments accurately reflect resident condition, services, and care needs
  • Lead or participate in PPS, Medicare, and utilization review meetings
  • Ensure timely completion, validation, and transmission of all MDS assessments
  • Collaborate with therapy, nursing, and interdisciplinary team members on ARDs and care plans
  • Monitor and analyze Quality Measures and initiate performance improvement actions
  • Provide ongoing education and support to staff regarding the RAI process
  • Ensure documentation supports regulatory compliance and optimal reimbursement
  • Prepare for audits, surveys, and regulatory reviews
  • Participate in process improvement initiatives related to quality and workflow

Full-Time | Exempt | Day Shift

We are seeking a clinically strong and highly organized MDS Coordinator (RN) to lead and oversee the resident assessment process. This role ensures accuracy, compliance, and coordination of the MDS, CAAs, and care planning process across all payer sources. This position is ideal for an RN with advanced clinical judgment and experience managing the full RAI process in a skilled nursing setting.
