Signature HealthCARE Bremen is seeking an MDS Candidate RN to perform resident assessments, gather data from medical records and interviews, and support PPS and Medicare processes. The role emphasizes detail, organization, and bedside involvement with families in a resident-centered setting.

Ideal candidates hold a current RN license and have at least 3 years of clinical experience in healthcare, preferably in long-term care.

Qualifications

• Detail oriented, organized, and self-motivated candidate for MDS work.
• Familiar with nursing and medical practices and long-term care regulations.
• Experience with MDS processes and Medicare/PPS guidance is preferred.

Responsibilities

• Collect information to complete the MDS using medical records, bedside assessments, and interviews.
• Participate in Daily PPS meetings, weekly Medicare meetings, and month-end reviews to ensure billing compliance.
• Educate interdisciplinary teams about the RAI process and changes.
