Job summary

Diversicare is seeking an MDS Coordinator/Clinical Coordinator (RN) to join our team in ensuring accurate and compliant MDS assessments. You will oversee the RAI process, coordinate with an interdisciplinary team, and drive high-quality resident care across the center.

Ideal candidates will have an active RN license, MDS/RAI experience, and strong leadership and communication skills to guide the team through regulatory requirements and care planning.

Qualifications

• Must hold a current RN license in state of employment.
• Experience with MDS/RAI workflows and regulatory requirements is preferred.
• Strong leadership, collaboration, and project management abilities.
• Proficient with computer systems and able to navigate multiple software platforms.

Responsibilities

• Coordinate the RAI process and MDS submissions.
• Ensure accurate, timely MDS assessments per state and federal rules.
• Lead care coordination meetings and ensure interdisciplinary collaboration.
• Educate center teams on the RAI process and documentation standards.
• Communicate care plans, discharge planning, and post-discharge steps.
